The State of New Jersey, Division of Pensions and Benefits issued both my wife and myself 1099-R forms for 2021 and the boxes do not match up with Turbo Tax:
there is no Box 15;
there is no Box 16; and
there is no Box 14.
Those boxes are for state tax withheld, state payer number and state distribution. Since you didn't have any state tax withheld, you do need that information, so I assume that is why it was not reported to you.
If the pension benefits are taxable in your state, that number will be picked up in TurboTax from the entry in box 1 and/or 2(a) and entered on your state tax return.
The Ohio Public Emp Ret Sys 1099-R only has 13 boxes. Box 12 is State tax withheld and box 13 is the State Payer's State Number. Turpotax has box 14, 15, 15, and 16. Box 16 being State Taxable Income,
My 1099-R does not have these additional boxes.....however the distribution is taxable for State. I have enough information to complete the boxes but it does not match the 1099-R. Help Please.

When you enter the gross distribution in box 1 and taxable amount in box 2(a) those values will transfer down to your state tax return and be reported as taxable income, as the case may be. You leave boxes 14 through 16 blank in TurboTax.

If you don't have any state income tax withheld in box 14, leave boxes 14, 15 and 16 empty.
Your asking for vdates on line 11 and 13, but my 1099r has none.
If your 1099-R has no dates listed on lines 11 and 13, just leave the entry box blank as you enter your data. Do not enter a zero in any box.

My NJ State Tax withheld is reported in Box 12; line 13 shows NJ and below that with no Box number it has as optional, Account Number
NJ is apparently issuing non-standard 1099-R forms. Someone else noted this too
DON'T Blindly enter the $$ in box numbers.
Match the subject of each box to the Software's boxes before entering the $$ or ID numbers.
TurboTax uses the IRS approved forms and box numbering, and those must be used when filing yoru taxes.

It's past time for NJ to get with the program and update their forms. Call/write your NJ representatives and Senators, and complain loudly. NJ is doing a disservice to all their retirees by foolishly maintaining outdated 1099-R reporting forms. Get hold of your former fellow NJ retirees to do the same.
I'm having a similar issue with a U.S. Department of State-issued 1099-R (Foreign Affairs Retirement and Disability System), which uses the following boxes:
Box 12: State Tax Withheld
Box 13: State/Payer's state no.
Box 14: State Distribution
Box 15: Health Benefits
Box 16: Long Term Care
Box 17: Dental Vision
TT appears to have uploaded the correct info for State tax, number, and distribution (into TT boxes 14-16). But how do I input the health and dental info from my 1099R?
You can enter amounts on your 1099-R for Health, Dental or Long Term Care insurance premiums in Box 5 on your 1099-R.
