There is no short form. Forms 1040EZ and 1040A were discontinued after tax year 2017. There is only the Form 1040 which is included in All the TurboTax editions.
Short forms---1040EZ and 1040A----have been gone since 2017 returns. Now everyone uses either a Form 1040 or a 1040SR ---if you were 65 or older at the end of 2020 you use a 1040SR which is identical to the Form 1040 but has bigger print.
A 1040 is the tax return form you prepare here to send to the IRS. If you start entering data in this software you are preparing a Form 1040. Please look at it so you will know what you are sending to the IRS:

If you are asking about the forms because you want to file for free:
You are posting from Deluxe $40
State return $40
Refund processing to have fees deducted from federal refund by 3rd party bank $39.99 ($44.99 in CA) (This fee is optional and can be avoided by paying TT upfront using a credit/debit card)
WHY DO I HAVE TO PAY? I WANT THE FREE EDITION
The information that you can enter in Free Edition is pretty limited now. Thanks to the new tax laws that began for 2018 returns, there are no more simple Form 1040EZ or 1040A's. Everything goes on a Form 1040 that has three extra "schedules" with it, and if you need any of those schedules, you are not able to use the Free Edition. Using the standard deduction instead of itemizing does NOT mean you will not need any of those schedules.https://ttlc.intuit.com/questions/4511011-what-happened-to-the-1040a-and-1040ez If you started in the Free Edition and entered any data that required any of those three schedules, you have to upgrade to a paid version. The Free Edition cannot be used for self-employment income, investment income, rental income or a student loan interest deduction,

You may be referring to a question regarding 2019 SE tax.
SE Family leave credit is for self-employed persons who did not go to work for up to 50 days due to becoming ill, or having a family member they cared for becoming ill.
The next question is about Schedule SE. When filing as self-employed on your individual tax return, the self-employed income is reported on Schedule C.
To answer the question if you used the SE Short form or SE Long form, you will need to get the 2019 Tax Return PDF if it is not already printed. Scroll through looking for your 2019 Schedule SE with Section A (Short) or Section B (Long) filled out.
This is all done in the Deductions & Credits section under Tax relief related to COVID-19. To return there, from the left menu, select Federal. Across the top, select the second tab, Deductions & Credits. Scroll down to Tax relief related to COVID-19 and start or revisit the applicable sections.
For more information, see: COVID-19-Related Tax Credits for Paid Leave Provided by Small and Midsize Businesses FAQs, which provides Overview of Paid Sick Leave Refundable Credit and the Family Leave Refundable Credit.
