Is it both capital gains and total dividends and distributions together or just one?

if you received a 1099-Div, there may be a portion related to US Government Investments. This may be noted in Box 12, or you may need to calculate the amount.
Here's more info on 1099-Div Government Interest (reported on Schedule B).
You may have Capital Gains from selling an investment reported to you on a 1099-B (reported on Schedule D). Capital Gains Distributions are not US Government interest because they are gains and not interest.
These are not added together to determine US Government Income, so If you can better clarify what you are asking, we'll try to help.
Sorry, let me clarify. I have 0.04% in U.S. government income that is in a security for my mutual fund. For taxes, I need to report amountwise U.S. government income I have received from this security percentage 0f 0.04%. With that being said, do I need to take that 0.04% and multiply it by capital gains, or just total dividends and distributions, or would it be both added together and then multiplied to get the amount?
No, the adjustment applies to Interest and exempt interest dividends only, not to capital gains. Click here for more information.
Q. Do I need to take that 0.04% and multiply it by capital gains, or just total dividends and distributions, or would it be both added together and then multiplied to get the amount?
A. Just the dividends, box 1a of the 1099-DIV.
In particular, the capital gains distribution, shown in box 2a is NOT multiplied by the 0.04%.
I should have noted, the 0.04% ist just for one security I have that has U.S. Government income. My box 1a on my 1099-DIV shows all dividends and distributions to the best of my understandings.
Wouldn't I just take the 0.04% from that security's dividends alone that I would calculate?
Q. My box 1a on my 1099-DIV shows all dividends and distributions to the best of my understandings.
A. Yes, some financial institution do it that way, while others issue a 1099-Div for each security.
Q. Wouldn't I just take the 0.04% from that security's dividends alone that I would calculate?
A. Yes
