When mailing the Federal return, include your 1040 without state forms. Attach a copy of Forms W-2, W-2G and 2439 to the front of Form 1040. Also attach Forms 1099-R if tax was withheld.

For the address to mail to, please reference this link: https://www.irs.gov/uac/where-to-file-addresses-for-taxpayers-and-tax-professionals-filing-form-1040

However, the IRS doesn't allow e-filing state returns without the Federal so it will not be possible to mail your Federal and e-file your state.

If you choose to mail your state return as well, include your state income tax form as well as a copy of the Federal 1040 and documents necessary for it as well. If you need help with what to include, please comment back. You can find the address to file at on the Department of Revenue site for the state:

My accountant neglected to e-file/submit my 2016 tax return to NYS (I'm assuming he failed to submit federal also). I need to mail in my 2016 return, I am self employed. Do I submit the entire federal & state package to NYS? How can I determine if the completed forms are State or Federal?

Thank you for your support!

@Joannie4362 You "assume" your 2016 federal return was not filed?  --you should check first.  Request a free transcript from the IRS.  If they have one, then your federal 2016 was filed.  And are you not aware of either paying 2016 tax due or of receiving a 2016 tax refund?  If you are getting a refund there is no penalty for filing late.   If you owe, then your penalty/interest has been adding up since the filing deadline. After the IRS receives your return and any payment of tax due as shown on your 1040, you can expect a bill for the additional penalty and interest the IRS will calculate. And you need to seek recourse through the negligent accountant.

You can get a free transcript from the IRS:

https://www.irs.gov/individuals/get-transcript

As for federal and NY forms--look at them.  Federal forms say "U.S." on them.  New York forms say "New York" and have an outline of the state of NY on them.  When you print them out, the federal forms will print first, then the state forms.

All of the 2016 returns must be mailed.  They cannot be e-filed.

When you mail a tax return, you need to attach any documents showing tax withheld, such as your W-2’s or any 1099’s.  Use a mailing service that will track it, such as UPS or certified mail so you will know the IRS/state received the return.

For your state:
