State tax filing
When mailing the Federal return, include your 1040 without state forms. Attach a copy of Forms W-2, W-2G and 2439 to the front of Form 1040. Also attach Forms 1099-R if tax was withheld.

However, the IRS doesn't allow e-filing state returns without the Federal so it will not be possible to mail your Federal and e-file your state.
If you choose to mail your state return as well, include your state income tax form as well as a copy of the Federal 1040 and documents necessary for it as well.
