Hi. I have a quick question. On my IRA account, I need to know about Box 1, 5, 11, 12a and 12b. Thank you for your assistance.

On my form, my amount in box 5 is $10,813.99 and 12b has an amount of $578.29. I'm confused, please help. Thank you to all that's reading this. I look forward to your responses.

What form did you get? A 1099R?
