I googled Clay Electric, which seems to be a utility company in Florida. Why do you think a check stub from a refund from your electric bill would be entered on a tax return? And....if you have no other tax documents, what do you plan to enter on a tax return --- and why?
Who has to file?

It might be taxable income if you took a deduction for payments in the past like as a business expense. You might be getting a 1099 MISC for a refund this month if it is taxable. Tax forms won't be coming out until the end of January or maybe in Feb. So you need to tell us why you got a refund.
