If you have not received a 1099-G for unemployment benefits, you can use your own records.  However, you should receive some statement from the entity paying the benefits.

Is this unemployment that you received from a union, private voluntary fund, or as a state employee? Or is it just regular unemployment/paid family leave?

You can probably go to the state's unemployment website to obtain your 1099G for the unemployment compensation received in 2017.

To enter it, go to Federal>Wages & Income>Unemployment
