If the annuity was established from an investment in the federal TSP, the distributions will be reported on 2020 Form 1099-R with state tax withholding information shown in boxes 14, 15, and 16. If the annuity is a Civil Service annuity, it's instead reported on Form CSA-1099-R with state tax withholding information shown in boxes 12, 13, and 14. As mesquitebean said, you must select the correct type of 1099-R in TurboTax to allow TurboTax to present the entry screen with box numbering that corresponds to your form.
Make sure that you are making your entries in step-by-step mode of TurboTax, not in forms mode of the CD/download version of TurboTax. The box numbering in forms mode does not adjust to the type of form, so if you do look at entries in forms mode you have to go by the box names rather than the numbers.
1. I received both a 1099 R and a W-2. The W-2 was used for a lump sum payment I received for my annual leave. I did not work in 2020.
2. The form I received was OMB No. 1545-01199 Form 1099-R.
3. The form does not refer to itself as a CSA 1099-R.
4. Box 12 on Form 1099-R lists "State Tax Withheld". Box 13 is listed as "State/Payer's state no. Box 12 does not list FATCA Filing required. On 1099-R the boxes are listed from 1 to 13. I do not have any boxes after 13.
5. The boxes on the Form 1099-R for 12 and 13 do not match the boxes on the Turbotax Software.
6. I updated the software before I started entering my numbers and then after I found the problem with the mismtached boxes. Updates did not resolve the problem.
In response to your question, the software is preventing me from making the entry. If I could manually override it and revise the boxes, I would and I wouldn't ask question. I did use the step by step mode. Also, as far as I can determine, there is no choice on what type of form 1099-R to use. You are using the term "CSA 1099-R". This is not the term used on the form I received from OPM. The form refers to itself as 1099-R. How are you making a distinction with the type of 1099-R received when OPM doesn't make that distinction? Also I am not sure what you mean by a Civil Service annuity. The forms I received from OPM refer it it as a federal annuity,
If it shows state tax withholding information in boxes 12 and 13 on a form from OPM, this sounds like distributions from Federal Employees Retirement System (FERS) reported on a Form CSA 1099-R. Delete the 1099-R from TurboTax and reenter it, indicating on the Tell Us Which 1099-R You have page that it's a CSR 1099-R.

you are looking in the wrong place the tax tables say the tax on 55468 for HOH should be 6553. you are reading the tax rate schedule incorrectly. on that amount of taxable income, the tax is $6162 + 22% on taxable income over $53700. so only about $1800 of your taxable income gets taxed at 22%. there is no error to report.
No it doesn't work that way. It's not a flat 22% on the total. It is only 22% on the amount over the previous tax bracket. Here's the Tax Table. Page 9 shows 55,450 - 55,500 = $6,553 for Head of Household.

To elaborate on what VolvoGirl said, of the $55,468 taxable, the the first $14,100 is taxed at 10%, then next $39,600 is taxed at 12% and just the last $1,768 (actually $1,775 because of the stepwise nature of the tax tables) is taxed at 22%.
@dmertz Where do you find the rates on amounts? Is there a chart?
The tax brackets are graduated. It's 10% for the first $14100, then 12% for any amount over $14100 up to $53700, and then 22% for the amount over $53701 up to $85500, and so on. The tax was calculated correctly.

More info is needed. I'll assume you have 2019 desktop software (CD/download) installed on your computer. Is it the Windows version or Mac? And what OS version?
What kind of errors? Are they showing up in the Federal Review and Smart Check?

A 2019 return cannot be efiled, it has to be printed, signed, dated, and mailed.
After review of the information a screen is presented with the questions "WHY DO I OWE $$$$", selecting that query presents another screen with a list of questions.
The information listed under "WHY IS MY TOTAL INCOME $$$$" appears to be incorrect.
Selecting this option presents a description of the various sources of income as follows: TAXABLE IRA DISTRIBUTIONS of $$$$, SOCIAL SECURITY of $$$$, PENSION PAYMENTS of $$$$, AMOUNTS EARNED AT JOB $$$$, UNEMPLOYMENTS of $$$$, THE OTHER INCOME OF $ -9200.
This appears to be the $10200 unemployment deduction incorrectly entered as -$9200.
