This is not the term used on the form I received from OPM. The form refers to itself as 1099-R. How are you making a distinction with the type of 1099-R received when OPM doesn't make that distinction? Also I am not sure what you mean by a Civil Service annuity. The forms I received from OPM refer it it as a federal annuity,
