If the annuity was established from an investment in the federal TSP, the distributions will be reported on 2020 Form 1099-R with state tax withholding information shown in boxes 14, 15, and 16. If the annuity is a Civil Service annuity, it's instead reported on Form CSA-1099-R with state tax withholding information shown in boxes 12, 13, and 14. As mesquitebean said, you must select the correct type of 1099-R in TurboTax to allow TurboTax to present the entry screen with box numbering that corresponds to your form.
Make sure that you are making your entries in step-by-step mode of TurboTax, not in forms mode of the CD/download version of TurboTax. The box numbering in forms mode does not adjust to the type of form, so if you do look at entries in forms mode you have to go by the box names rather than the numbers.
