You do not enter a 1095B anywhere. You only enter a 1095A if you have marketplace health insurance. You do not enter a 1095B or a 1095C.
Then where do you enter that I had insurance as stated on 1095B ??
On a federal tax return you are not asked if you have health insurance. Are you being asked for health insurance on a state return?

The Form 1095-B is not entered on a tax return so none of the information on the Form 1095-B are entered on a tax return.
