If you lived in a traditional tax base school district, you’ll need to complete lines 19-23 on page 2 of Form SD 100. Enter the amount from line 23 on line 1.
If you lived in an earned income district, complete lines 24- 27 of page 2, and enter the amount from line 27 on line 1.

You can find your relevant school district numbers, tax rates and tax types under Ohio School District Numbers in the instructions linked above. Districts with tax type "T" use the traditional tax base. Districts with tax type "E" use the "earned income" tax base.

@Putz Did you use TurboTax to prepare and file a 2021 federal return? If so, you can go back in and use the CD/download software for 2021 to prepare the state or school district return. Otherwise, no, TT cannot just help with a prior year school district return. But you could seek the form from the district and prepare it by hand.
The school district tax form (SD100) is a state tax form. You get it from the Ohio Dept. of Taxation (ODT), not the school district (SD).
