Yes it will probably be 100% taxable and a 10% Early Withdrawal Penalty if you were under 59 1/2. Did you have any withholding taken out? And don't forget your state.
My understanding is that for 2020 there will be no 10% penalty and I'll also be able to spread the tax burden over 3 years. Is this correct?
That is correct if you made the withdrawal because you were negatively affected by COVID 19.

Only if the distribution was necessary because of COVID and you meet the requirements for a COVID related distribution.
And what proof does the IRS require? Is there some form you fill out?

About the tax withholding.....
The tax withheld in box 4 is only applied towards 2020. Therefore, you might want to adjust your withholdings in the next two years if you spread the amount over 3 years.

You always must be able to prove anything you put on your tax return if audited and the IRS asks for it, but not with yiur tax return.
No form. just enter the 1099-R into TurboTax and it will ask if it was a COVID related distribution and answer the questions.
So I've never amended a return before. Turbo Tax will fill out 8915-E for me and then submit it electronically to the IRS when I do the amendment? Thanks for all your help! I also forgot a 1099- NEC so I have to add that too.

These are the reasons that the IRS will accept (there could be others):
A3. You are a qualified individual if –
Under section 2202 of the CARES Act, the Treasury Department and the IRS may issue guidance that expands the list of factors taken into account to determine whether an individual is a qualified individual as a result of experiencing adverse financial consequences. The Treasury Department and the IRS have received and are reviewing comments from the public requesting that the list of factors be expanded.

Do not rush to amend - no hurry. You have 3 years to amend and amended returns are taking 4-6 months or longer to be processed so waiting a few weeks will make little difference and amending too soon might just compound the problem. (If you amend today you probably will not see the results until August or September).
To amend your 2020 tax return:
-- First, *wait* to see if your return has been accepted or rejected by the IRS or state. DO NOT do anything until you receive the accept or reject e-mail.
-- If rejected, you can correct and re-send your return.
-- If accepted you should *wait* until your return has been processed and you receive your refund or conformation that any tax due has been paid. (If you file an amended return while you first return is being processed it can cause extended delays for both returns if two returns are in the system at the same time). In addition, if the IRS makes any change on your original return, you might end up having to amend the amendment – a sticky process that can take a year or more).

It is suggested that it be mailed certified with return receipt (or other tracking service) to verify that the IRS receives it. That is the only proof of mailing that the IRS will accept.
-- Amended returns can be mailed or e-filed - allow 8-12 weeks - can take up to 16 weeks (4 months) for processing.
