If there is no amount in Box 2a (or 'taxable amount not determined' is checked) on your 1099-R, the taxable amount is the amount in Box 1.
If there is an amount in Box 5, this may be subtracted from the amount in Box 1 (indicating after-tax contributions).

Be sure to enter the 1099-R exactly as shown; including the Code in Box 7.
