As hfluss posted, add supporting details for the RE taxes (line 2a), list the total paid, and then subtract the 8829 amount. This worked for me.

You know that it is the same $5k just moved two separate times, but all the TurboTax program has to go on is that there were two separate Form 1099-R entered as part of your return. So, on screens summarizing your income for information purposes, the program just takes into account that $5k was distributed two separate times under your Social Security number. Therefore it does look like you have more 'income' for the year than you really do.
The important part is that your Form 1040 is accurate and only the portion of the distributions that should be taxable are being included in your taxable income.

Log Out Completely: Before doing anything else, click Sign Out in the TurboTax menu.
Clear Cache and Cookies: This removes old, "corrupted" data that might be confusing the website.
Use an Incognito (Private) Window: This is the most effective "quick fix" because it launches the browser without any saved history or extensions (like AdBlockers) that often interfere with tax forms.
Check Pop-up Blockers: TurboTax often opens forms (like the 1099 PDF) or payment windows in a separate pop-up. Look at the right side of your address bar for a small icon with a "red X"—click it and select "Always allow pop-ups from Intuit."
Restart the Browser: Close all open windows of your browser entirely and then reopen it.
Most TurboTax errors happen because the browser is trying to use an old "session" from a previous visit. By clearing the cache or using Incognito mode, you force the browser to establish a brand-new, secure connection with the Intuit servers.

I always recommend that when reporting employee stock transaction that the information be entered manually, as opposed to importing. You should enter the information on the 1099-B exactly as it appears on the form, and then adjust the stock price to account for the income reported in box 1 of your W-2. I don't know why the supplemental information on your 1099-B would have a $.064 Wash Sale listed if it's included on the 1099-B itself. The amount is so small and it's not on the actual 1099-B, so I would just ignore it (don't report it).
The information you enter for the transaction should transfer to your PA-40 Schedule D. It should also have the same gain or loss. If for some reason it does not show the correct adjusted loss of $1.76 you will need to make an adjustment in the state tax section. Unlike many states, Pennsylvania individual tax returns do not start with federal adjusted gross income (AGI) or federal taxable income. Instead, each specific class of income has to be reported directly on Form 40. The ordinary income ($21,067) from the exercise and sale of the NQSO should also have been reported in box 16 of your W-2. That would then flow to Form 40 line 1a. The loss of $1.76 should be on line 5.

If you received a "Success" message but your data hasn't appeared in TurboTax, it is likely due to recent browser security updates regarding Local Network Access.
To fix this you need to allow TurboTax to securely retrieve your data through your browser settings:
Locate the View Site Information icon (the "tune" or "lock" icon) on the far left of your address bar, just before the URL.
Find Local Network Access in the dropdown menu.
Toggle the switch to ON.
Refresh the page and attempt your import again.
What do I do when my data isn't imported into TurboTax Desktop even though it says successful import in my browser?
If permissions aren't the issue, one of the following factors may be at play:
Brokerage Delays: While most forms arrive by January 31, many brokerages delay Consolidated 1099s (like the 1099-B) until mid-February due to complex reporting requirements.
Availability: Your brokerage must specifically "release" the import file to TurboTax. If it isn’t ready, we recommend checking back.
Connection Blocks: Active VPNs or aggressive Anti-Virus software can occasionally block the import feature between your bank and TurboTax. Try disabling them temporarily or switching to a different browser.

If your box 2a has a non-zero number in it..(not blank, nor marked as undetermined), and that number is somewhat lower than the box 1 number, due to an after-tax contribution amount that is noted in box 5...then edit that retirement form again, and when you get to the following page (below), answer it as-if the displayed edits were present (i.e. you needed to answer YES). Answering YES will use the box 2a $$ amount on your for 1040. Checking lines 5a and 5b on your form 1040, before and after entering that 1099-R, and the 5b amount should be lower than line 5a. (But if you answered No, and already entered some $$ in the "General" or "Simplified" calculation area ,you might need to delete your 1099-R first and re-enter it from scratch...not sure of that though) __________________________

Answer Yes
Continue to the form
Delete it using the trash can
Repeat the process

Code T is for employer provided adoption benefits. However, in order to claim the adoption credit or exclude those benefits from taxable income, the expenses must relate to the adoption of an eligible child. Sperm does not meet the definition of an eligible child. Since you mention there is not a surrogate, I am assuming this is for IVF treatment. IVF treatment may fall under medical expenses which could be claimed for itemized expenses (if already included in taxable income), but not the adoption credit.
Since your employer is refusing to issue a corrected W-2, (I'm assuming box 1 has been lowered by the amount in box 12) you can take the following steps to make this taxable income.
You should contact the IRS who may then send a letter to your employer. The IRS will send you information on form 4852 to fill out, which you can do in TurboTax following the instructions below.
To fill out form 4852 take the following steps:
Enter the W-2 as you believe it should be which would mean adding the amount in box 12 code T to the amount in box 1.
Select continue
In the screen the says Let's check for other situations select Didn't get a W-2 or received and incorrect W-2
Answer yes to do you still want to complete a substitute W-2
Continue through and answer the questions
You will need to explain what you did to get it resolved the your employer and also how the calculations are incorrect

To fix the issue with your Idaho state return not showing in TurboTax, follow these steps: 1. Download the TTCleanStatesTool.zip file for your tax year from TurboTax support. 2. Extract the contents of the zip folder to your desktop. 3. Right-click the TTCleanStates.exe file and select Run as Administrator. 4. Select your TurboTax version (Personal or Business). 5. Choose Idaho from the dropdown menu and select Remove. If you have problems with multiple states, you can choose Remove all States. 6. After removal, close the tool and reopen TurboTax. 7. You can check to see if it was part of your return by looking at all forms and schedules. Here's how to do that.
TurboTax Online:
Sign in to your TurboTax Account.
Scroll down to the "Your tax returns & documents" section.
Select the specific tax year you need (e.g., 2025).
Click "Add a State" (you aren't actually adding one, but this "unlocks" the return so you can access the Print Center).
In the left-hand menu, select Tax Tools > Print Center.
Select Print, save or preview this year's return.
The Key Step: Look for a checkbox or dropdown that says "Include government and TurboTax worksheets" or "All forms."
Click View/Print to generate the full PDF, which will now include every schedule (like Schedule C, D, or E) and the internal worksheets.
TurboTax Desktop:
If you used the software installed on your computer, you must open the specific program for that year (e.g., open the TurboTax 2025 app).
Open your saved .tax file.
Go to File > Print.
