I am trying to input my personal contributions to my HSA. This was after tax and after payroll but I cannot get to form 8889 to input it. Does anyone know a work around to input personal contributions after-tax to their HSA? I have gone through the HSA prompts 3-4 times and I do not see a way to show that I made personal contributions separate from an employer.

You can use the session method and enter your manually calculated session totals into TurboTax.
Instead of entering every W-2G exactly as written, enter your Total Net Winnings from all "winning sessions" as a single entry.
In TurboTax, all W-2G income is entered with Other gambling income (not on a W-2G) and then the losses are entered on the very next screen.
Comprehensive Record-Keeping: You must maintain a precise, contemporaneous diary or similar record of all wins and losses.
Supporting Documentation: You must be able to provide tickets, receipts, casino statements, or other proof.
Limitation: While this "session" approach can help determine your daily net, overall for the year, you can only deduct losses up to the amount of your total reported gambling winnings, and you must itemize deductions on Schedule A (Form 1040).

Box 12 with code "J" would list the amount of your sick pay that was not included in boxes 1, 3 or 5. So, it may be that the W-2 is not listing your disability pay amount in box 12. However, that would not likely affect your tax since the correct taxable income should be listed in box 1. I suggest you enter the amount of the disability pay in box 12 and see if your tax changes. If not, then you can leave the box 12 entry off of your W-2 entry.

If you need to update any of this information, here's how:
Sign in to your Intuit account.
To update your user ID, password, email address, or phone number, select Sign in & security.
To update your name, date of birth, address, or occupation, select Profile.

Yes. If it shows a start and ending date, then it is likely that you did file as part year. Unlike some states that have a specific form for residents, one for non residents and another for part year residents, there is only one main form for PA, whether a resident, Part year, or Non-Resident so the box that you were a part year resident would have been checked if you saw a -to- date.
