Arizona Form 140A is the resident tax return short form. It can only be used if your income is less than $50,000 and you don't have any adjustments. It is 3 pages long, so it doesn't sound like what printed out. Without know what the questions are or what the form actually is, it's hard to say for sure whether you should ignore it.

Arizona Revised Statutes § 43-1042 was amended to require taxpayers to reduce the amount
of itemized deductions for amounts used to claim an Arizona credit even if the amount
was deducted on the federal return as state income taxes paid rather than as charitable
contributions.

Based upon the numbers provided, you may not have a filing requirement in Arizona. For 2024, individuals are required to file an income tax return if their Adjusted Gross Income (AGI) is more than $14,600 if single or filing separately, $29,200 if filing jointly, or $21,900 is filing as Head of Household. If your income is below these limits, then you would not need to file an Arizona return.
However, since Arizona reports income in the same manner as your federal return, if your AGI on your federal return exceeds these amounts based upon your filing status, you would have a filing requirement in the state.
