I had to print state of arizona return to mail in. When i printed it gave me a blank 140 a form to complete, not sure why? it has question 1a-7a. can i disregard this?

i itemized federal return and efiled, the state needed to be mailed in, but i got confused by this blank form and why it would be given to me to fill out since i though turbo tax would complete that? can i mail it blank since i dont know how to fill it out?
