I requested the form twice via the website. No way to get an actual person on the help line. Not sure what to do. 10/15 is coming up fast!
You will have to enter a 1099G that is issued by your state.
Some states will mail out the 1099G. Or you might need to go to your state’s unemployment website and use the password, etc. that you have been using to certify for weekly benefits to get your 1099G from the state’s site.
Enter your 1099G in Federal>Wages & Income>Unemployment
Go through the screens very carefully, making sure to enter any federal/state tax you had withheld from the unemployment.
You have until 10-15-2021 to e-file a 2020 tax return. After that e-filing will shut down and your return will have to be filed by mail.
Just file your taxes as if you never received unemployment. It's that simple. I received unemployment and it was taxed for the whole year of 2021. I haven't received my 1099-G and the unemployment website section where I SHOULD be able to view it doesn't allow me to. There is another website for the state that is supposed to allow you to view it if you provide your SSN, Last Name, Zip-code, but that is also not working. My last option is just to file my taxes. It's just the IRS, who cares. They'll just take it out of next years taxes if you owe anything. The IRS is owned/created by the Rothchilds anyway. When you think it's the government you're paying, well.... you're paying the same people that the government borrows money from. The Rothchilds. F'em.

You can request your 1099G from Florida at the Florida 1099 G Request.
You can also wait to see if they mailed them out. They have until January 31, to mail out the forms. After they mail them, with the current mail delivery times, it may take 1-2 weeks to receive.
I have called unemployment 5 or 6 times to let them know I have not gotten my 1099g for 2021
To clarify your question, which state paid unemployment to you?
If I remember correctly, when you're in the website and page of where you can get your 1099, check up around right hand corner of that page and look to turn off cookies and that will allow you to see your 1099 and be able to print it out. Hope this helps.

How do I report unemployment if I didn't get a 1099-G?
Florida changed the login requirements for their website since I last signed in and I can't login anymore.
I've requested my 1099-G from the Florida unemployment site no less than 6 times. They just don't send it. I've asked for them to send by USPS and email... nothing. Zero response of any kind.
@Soundmn That is very frustrating. But you still are required to get hold of your 1099G and enter it on your tax return, despite the frustration of wrangling with the unemployment office. If necessary, get an extension to file your Form 1040 and pay estimated tax by April 18, 2022. That will give you more time to deal with the 1099G.
