TurboTax says I have additional Medicare tax and should upgrade to Turbo Tax Deluxe. But I don't have form 8959 and I don't want to upgrade.

If you are only reporting income from a W-2, is the amount in box 6 1.45% of the amount in box 5?

Even if the Medicare tax is off by $0.01 cent the program will require the Form 8959.  Try reducing the tax withheld by $1.00.  Note that Medicare taxes are not reported on a tax return.
