There are 2 other people on my 1095A form. We each file taxes separately and claim an equal percentage of the plan (33% each). Should I leave the SLCSP percentage blank?

No, if you are claiming 33% of the SLCSP, you will enter the 33%. Otherwise you will not have anything allocated to you on the tax return.
