Your income from Lyft can be entered in TurboTax without the address and EIN.
From the Self-employed summary, select Add income from this work. Then, under COMMON INCOME, select Other self-employed income, includes 1099-K, cash, and checks and Continue. There is only a description line here, no EIN requirement.
