I received a "Request to Confirm Tax Return Filing" from the State of California (form FTB 3904). What does this mean?

I just got this notice too.  As others have stated, it asks you to send copies of literally every form of ID you have.  I'm not comfortable putting this stuff in the mail!  The statement above doesn't seem to match my experience either (my federal return has already been paid so it must not have looked suspicious to the IRS), and the addresses, form names, etc. are all different on the form itself, the phone message and the website.  I don't know what to do.

and called the number. They asked me a couple questions, including my SSN number and they told me I dont need to send those docs anymore. Everything is fine now.

Weilin_Chao, wow! thank you so much for posting your comment.  I went to the website too, and called the number.  I was also able to provide my SSN and a few other details from the return, including the direct deposit account number, and was also told that I don't need to send these docs.  My return is now being processed.   Whew, what a relief!  Thank you!

I received the same letter. I called the number, gave some information,  & was told I didn't need to send the documents requested.  I was told refund is on the way!  Huge relief!

I got this too, and I'm wondering if it's because I asked to have my refund deposited to my brokerage account for the first time. All previous years I had it deposited to my regular checking account.

That wasn't the response I got. I called that same number ending in 7088 and the lady asked me to send in the documents (or fax) just like the letter requested. I still don't feel comfortable doing this. Did I just get unlucky and get someone who doesn't care?

FWIW, I was also told that I must send in all documents. I expressed extreme concern about sending personally identifiable information through the mail or fax. The rep said I could also FedEx to:

Franchise Tax Board
Sacramento, CA 95827 - 1500

and FedEx will know where to deliver it. Or, register for a MyFTB account on ftb.ca.gov and the documents can be sent securely (I am skeptical of that but I'll give it a try).

Thanks all for the info here, I was able to call and a lovely woman straightened it out for me in minutes. In my case, all the information was correct, but she indicated sometimes the state does not have all of the withholding information at the time of processing and therefore spots discrepancies that aren't there.

I just spoke to a very nice lady who asked for a few details and cleared my refund straight away. I'm guessing the reason some people can't get cleared might be because they have more significant "red flags' on their accounts.

Received the same MS-151 form. Got the un-lucky draw and will have to send everything in. This comes across as a delay-to-pay/refund rather than a genuine identity concern. Very annoying.
