No GE 1099-DIV  Where can I get a replacement on line?

You don't have to report that the form 1099-DIV is a replacement. The IRS is not concerned about this at all.  Just enter the new form as 1099-DIV, as usual. If you don't have the form but have the dividends information, enter it as if you received the form:

1. Federal Taxes
2. Wages & Income
3. Interest and Dividends - select Interest on 1099-DIV

Are you saying you don't know who issued the 1099-div that you misplaced?

Yes. I cannot contact the issuer of the 1099--I need a replacement 1099 or the information on the 1099

Do you have a broker that handles your investment? He/she should be able to give you the information... If not, you have a few options... file for an extension and search for the form or contact information, order a transcript from the IRS, the transcript will include all forms that they have received.. and form 1099-DIV will be listed there,  OR file without the form and wait for the IRS to contact you in the summer. Once they see that the form is missing, they will send you a correspondence with the recalculated tax based on the 1099-DIV that they have.  And actually, you might not have to pay any tax- if your ordinary tax rate if less than 15%, you may pay zero tax anyway.

You have to contact the issuer of the form for a replacement.

If it is from your bank account or investments you might be able to log into that account and download another copy of the 1099Div.

As mentioned by others, if you hold the shares in a brokerage account, contact your broker. But it sounds like that be the case. If you hold the shares directly, you might have luck contacting GE's transfer agent. See
