When filing by paper, do I need to include all worksheets?

Specifically, the qualified dividends and capital gain tax worksheet associated with Schedule D.  I printed my federal return to file by mail due to an unrelated compliation but there were no worksheets included.

No, you do not mail worksheets, only the tax forms and in the order listed at the top right corner of the tax form. 

You may want to print a copy for your file and include the worksheets.
