I don't have a physical form so I'm not sure what to enter in for the boxes "Received From", "Box-1 Interest Income", and "Box-3 - Interest On U.S. Savings Bonds and Treas. Obligations".

Yes report anything over 50 cents (rounds up to $1). Enter it like a 1099INT and put the amount in box 1. Just enter the bank name.
