If I made under $10 dollars from my bank savings and I did not receive a 1099-INT from my bank, do I still have to report it? If so, how should I report it?

I don't have a physical form so I'm not sure what to enter in for the boxes "Received From", "Box-1 Interest Income", and "Box-3 - Interest On U.S. Savings Bonds and Treas. Obligations".
