If you e-filed your state return and where suppose to mail anything in you can find what it is that you need to mail by opening up the PDF version of your tax return, before printing, and you will see an instruction page with what you need to print and mail. I have provided instructions below on how to download and print the PDF version of your tax return:
I received an email exactly like this as well, but when I printed out my PDF version of my 2020 tax return, there were no instructions or additional details as to what forms I needed to mail in for my State tax return. Any advice would be greatly appreciated.

Same exact issue. I also got an email saying my state accepted my return but that I have an additional form that needs to be mailed in. I followed the instructions from the email and this post, but when I download the PDF copy of my 2020 return and/or print it, it does NOT contain any pages with additional instructions about what to print. How do we know which form needs to be printed and mailed to our state??

I have the same issue. My email says extra form needed after my state was already accepted. But the pdf doesnt show any instruction on what i need to send.

Your best option is to contact your state's taxation department at the phone number they provided on your notification. Write down the information they provide, and if you can provide us with the name of the form in question, we can provide additional assistance on how to find the form they are asking for.
