Form 1040 line 11 Schedule 8812

I am single, have no children, and in the review, two errors were found in Schedule 8812 / Form 1040 line 11 which refers to credits for qualifying children and dependents.  Why was Form 1040 attached to my return? Is the CD defective?  HELP

Please follow the steps below to fix the error:

1. Login to your account.
2. Go to Federal Taxes
3. Go to Deductions and Credits.
4. Scroll down to You and Your Family and click Start next to Child and Other dependent Tax Credit
5. Click continue and on the following screen We'll start off with advance Child Tax Credit payments select No.
