I have a friend that received a Form 1099-R, code 3 for Box 7, she got separarated from her husband last year, and he is deceased as well. To her understanding he removed her as a beneficiary. She usually doesnt file taxes because she is retired, but since she received the form, what can she do? Thank you. Any information given will be appreciated.

did she get the money? code 3 is for disability not death. the code may indicate the distribution was made before he died and that his spouse was a beneficiary at the time. only the custodian can provide complete info as to why she was listed on the 1099
No, she did not received any money, somehow when she called she was told that he removed her as the beneficiary. So I don't know why she received the form, is it because they are legally married? or because she was listed as a beneficiary? Thank you so much
As Mike said, these are questions for the custodian of the retirement account. It would be odd that they sent the money to one party but the 1099-R to another (was the 1099-R in your friend's name or the late husband's?).

The form is under both, first its her deceases husband and she is listed as second.
If she need to file, for example, will she need to report the whole amount or only half?
Who's SS# is on the 1099-R ? That is the person who has to report it on their return.
Its the deceased husband SSN.
Then it goes on his final return or the estate's return ... whoever is responsible for filing these returns should handle this.
