Form 568 (California) no longer inputs EIN number

I am on TurboTax software.

I am comparing it to my tax return that I did on TurboTax last year.

California form 568 (LLC) should include both SOS and FEIN number.

my last year’s 568 included EIN (FEIN) number properly.

this year when TurboTax generates form 568, it leaves the FEIN field blank (it inserts SOS correctly). I’m not able to insert the value for FEIN when I go to form view.

for reference - it is in the very first line of form 568. The format is:

SOS company name FEIN.

(example:

[phone number removed] COMPANYNAME [removed])

however, this year the last value is blank, and it is an important one.

I see that I added it when prompted in Easy Steps.

please advise how to insert it into the form

please advise how to add it on the form.

Any update on this?

I hope my question makes sense.

When I generate PDF tax return package, form 568 doesn’t show FEIN on it.

but the form requires it per image:

I see that it is filled out on my worksheet for 568, but on the form generated in the final PDF package,  FEIN is blank

Which version of TurboTax are you using? It sounds like you are using a Desktop version.

I just tested this with both TurboTax Premium Online and Home & Business Desktop, and the EIN entered in Schedule C did populate on Form 568 in the California return when the California interview questions were answered. I tried printing it to PDF both in step-by-step and Forms mode.

However, there were some instances where I tried to print and the top portion was blank  as in your screenshot.

I suggest giving it another try.

I am on desktop version, Deluxe edition.

I see FEIN on the LCC payment voucher (form 3522), but not at the top of the next page - which is form 568.

the very first line of that form should have:

SOS number - first four letters of the company name - FEIN number (no “-“ in between).

and that’s how it shows on my TurboTax return from last year.

foe this year though my first line only prints SOS number and the first four letters of the company name.

next line shows

TYB 01-01-2024 TYE 12-31-2024

hope this makes sense.

Also to add - when I go to see ALL forms, and I look at form 568 in there, it looks different.
it has a section called “Additional Information for Scanband Smart Worksheet”.

in that section the EIN number shows correctly in “Employee ID Number” field.

however, it doesn’t show on the first line of 568,l in the view that is being sent to the IRS.

i hope this makes sense. My understanding is the additional information section part doesn’t go to the IRS.

Form 568 is a California form and would not be sent to the IRS, but to the California Franchise Tax Board.

See screenshot of the top of the printed form below:

This is how the top of the form looks when printed from TurboTax. The EIN is shown at the top where highlighted.

I posted instructions to print with more screenshots in your other thread.

Form 568 (California) no longer inputs EIN number

@MonikaK1 correct, this is where EIN should show.

that is exactly the spot where mine is missing.

Thank you for posting instructions - I followed them but the same result - where you see "33...." I see a blank field, and I am not able to manually fix the form.

Again - I see my EIN on all other documents - even on the worksheet for form 568

Tip: when posting questions involving how forms appear in TurboTax, always specify the version you are using and whether it is Windows or Mac.

I have now gone to my Mac and added a Form 568 to a test return. I requested the California return print to PDF. The layout of the Form 568 looks different on the Mac version, but the EIN I entered on Schedule C appears on the form, just about the TYE date with no hyphen.

Make sure you enter the EIN in the Schedule C in the Federal section. From there, it should flow through to the CA Form 568.

You might try saving and exiting the return, exiting TurboTax and reopening the return. Go back to the Income section and delete each investment income item that's not needed.

See this help article for troubleshooting steps for Mac issues.

If you confirmed that you have all of the minimum system requirements, you might also try clearing your cache and cookies.

If after trying all of the options mentioned you still have the same problem:

Form 568 (California) no longer inputs EIN number

@MonikaK1  I did specify multiple times that I am on desktop deluxe edition, Mac.

I also chose Mac from the dropdown when prompted during publishing of this post - looks like that information doesn't carry through?

Yes, as I have stated before - EIN is definitely on my schedule C, line D.

It is also on 568 worksheet under Employee ID.

But it doesn't carry over to 568 itself.

I don't have any investment income.

Deleting all of my business income is going to be a pain, and I am not sure how it would help - EIN in Schedule C, and I see it entered in Easy Steps when I was prompted for an Employee ID.

I downloaded desktop edition on my windows computer and opened my taxes there. Triple checked - EIN is on Schedule C, deleted and reentered 1099-NEC income.

still - form 568 is missing EIN, worksheet for 568 shows EIN.

i called the number on the link that you have provided - a bot answered and kept asking me to pay for Live Tax support.
no way to get to a person to help fix what a technical issue.

how can I get through to someone?

when I had TurboTax online last year, someone was able to remote into my computer and help me.

now I am on PC, TurboTax Desktop Deluxe edition, restarted, updated, etc..

Just to share more info on my investigation hoping this will be useful for others.

i called TurboTax, was able to get to a person who then said they aren’t sure why it is not carrying through and forwarded me to a tax expert. But at that point the bot would not allow me to proceed without paying $60.

At that point I used ChatGPT.
It said this is actually an known issue with TurboTax for single-member LLC.

It also had me check if my EIN is generated on 568 income worksheet that gets filed with 568 - and it is indeed generated on the top of every page of the worksheet.

chatgpt concluded that I should be okay to file as is.

below is the response.

And here is some more context from ChatGPT

@MonikaK1  are you using a single member LLC in your testing? (no rental income, by the way)

Yes, I was using the single-member LLC situation for the test. Thanks for sharing the additional information you found. Sounds like you are good to go.

Form 568 (California) no longer inputs EIN number

@MonikaK1 I’m not sure if I am good to go.

I just shared what ChatGPT thinks. But when I read FTB’s requirements for form 568, they state that EIN is required on it.
Do you think it’s okay for me to submit the form without it?

I am not able to reach a live person who can help me without on TurboTax phone line unfortunately.
