1099Q received has my husband listed in recipient box but my daughter's SSN listed. Can I use this form and who should report? Payments went direct to university

Wondering if I need to request a corrected form?  According to IRS, it appears if payments went direct to school - beneficiary should report but not sure since name and SSN don't match on my form.

Yes, you can use that form. If it is used to apply against your daughter's education expenses, use it on your return along with the 1098-T. If it will be claimed as taxable income, your daughter will claim that amount on her return. 

If you are applying the total distribution to the tuition paid, it won't be taxable, and it won't be reported. You are right, if your husband is the owner of the account, and your daughter is the beneficiary, the distribution should list her name. You may wish to contact the financial entity that handles your account for clarification, but since the IRS matches forms to Social Security numbers, I would not wait for a corrected form in order to file.
