My case is the following: I have lived in the US for 10 years. I had J1 for 2 years, F1 for 3 years, H1-B for 3 years, and now F1 again. I filed for the past 4 years as a resident and I think I should be able to continue doing so. I received a 1042-S with income code 16 in box 1. There is no exemption code and not tax treaty. I am from Colombia. This money is from a fellowship, but I am puzzled by the code. How do I report it in form 1040?

You would enter it in the credits/deductions section under educational expenses (yes, fellowships go there too). If you are still a degree student, you can enter related expenses to reduce the taxable part of the fellowship.
Since there is no tax treaty, that is really the only thing you need to do, unless you had taxes withheld, enter them under estimates and other taxes paid.
I would recommend to file by mail to add the copy of the 1042s, so the IRS can match that 1042s to your return (as they get a copy from the payer as well).
