where does the $number on Line 7 of Form 8889 come from?

Basically, you multiply $1,000 times the number of months that you were an eligible individual. This is for individuals who are 55 or older.

 

You can see the IRS instructions in the 8889 Instructions on page 7.

 

However, if you are not married, then the additional contribution appears as part of Line 3. Why is not clear, but see the instructions for Line 3 in the same publication.
