I have a few questions regarding Form 1099-R, box 15. On this form that I received from my bank, box 15 is blank. (Box 14 does show the state tax that was withheld on the IRA distribution.) Box 15 asks for the (1) State and the (2) Payer’s State No. TurboTax says these two fields must be filled in in order to complete my tax return.
I will contact the bank to try to get this information. I just want to be prepared.

1 - The state in box 15 is the state in which you reside.
2 - Yes, he payer id no. identifies the payer, not necessarily its location. However on one of my 1099-R the number is not shown and apparently TT accepts this.
Yes, Box 15 State ID refers to the State that received the state tax withholding (usually the taxpayer's state of residence). Enter your state's two-letter postal abbreviation (ex: CA for California).
The Payer's State No. is a number assigned by the State to the source of the retirement distribution (typically a bank or brokerage). Most state programs do not pick up any state codes entered in this box.
Entering a payer number in the second box may be important if you're planning to file the State return electronically. You'll be asked for this number during the State review section if needed. But if you are filing a paper state return with either Minnesota or Virginia, you will need to enter the payer's State Identification number. These states require this information to complete the return.

I now know what should go in the first field in Box 15. The state where I reside and the state that received the withholding are the same in my case.
However, I am still a bit uncertain about the second field in Box 15. I am getting the impression that the Payer’s State No. depends on the state that is assigning the number. Is it that state that received the withholding, or is it the state where the bank is incorporated (the two are different in my case)?
Robtm: Turbotax indicated I had to fill in Box 15 when I went through the Federal review process. I was hoping I wouldn’t get flagged for the missing information.
I almost regret contacting the bank. I haven’t received their promised reply yet (the representative at the branch didn’t know the answer). Now I have to wait (and wait) to see if they will issue a “corrected” Form 1099-R, which I would have to account for in TurboTax.

Hi I was hoping there was going to be a answer for the topic...I was about to get excited...I will come back with an answer if I can get my clients error message concerning this topic resolved ...
we entered VA and then entered the Payer ID number in the box that was listed on the 1099R the software then stated a error message ..the following question stated: we had entered the state information twice and to remove one and we put one of the entrance in the garbage can...the software never gave the error response that we entered the state twice until we put in VA and Payer ID number in the 2nd Payer box (the error was corrected after the entrance were made after we hit continue to software stated 2 entrance of state ....the client never indicated that he lived in another state in the questioner to trigger the state being listed twice on the 1099R and causing the error message in the federal review
