Does anyone have a template or form (example) of the legally blind document that’s required for a doctor to fill out for proof? Please and thank you! -Hannah

Where are you being asked for that?  There is nowhere on your tax return that requires you to enter such a form.  You would only need that if the IRS requested it.   When you prepare your tax return, you can say in MY INFO that you are legally blind, and you get an extra amount added to your standard deduction.
