Do I have to use a W-2 with no income (Box 1 blank) but box 12(a) has code DD (Medical) and amount? Can I ignore this W-2? How do I file electronically if I have to use?

TurboTax says that I can't file electronically (IRS regulations) if Box 1 is zero.  It is actually blank but TurboTax puts in a zero.  DD is for company paid medical insurance which is non-taxable.

You don't enter it for just DD in Box 12.  Just keep it for your records.
