In the pdf and in the printed output, the D-400 form is askew. Most field-names are missing, but the data is there - so you have data but no clue what Line# it belongs to. When will this be fixed? (assuming I cannot submit it like this)

I didn't have any issues using W10, and the Premier Desktop software. And Adobe Acrobat DC reader.

I have three tax returns in one pdf file: Federal, Wisconsin, and North Carolina.
Only the North Carolina is askew. The other two are fine.

Strange.....IF you can, try creating a PDF of only the NC tax return, and see what happens then.

Something may have gotten garbled in the creation of the PDF just before the NC section was created.
Then do the full PDF again

Wow....don't know. Now I'm grasping at straws...BUT
Shut down TTX...then
Check for any Windows updates again?
Sometimes, a waiting W10 update can screw up TTX.
Then check TTX software updates once that is done.
(I just checked for Acrobat updates too from within the program, but didn't have any....not sure when I ever checked that)
Ran Windows Update (even though it ran at 4:30am today), and there was a new update - took it. Restarted.
Chkd Adobe Acrobat, current version already.
Re-start TurboTax, no auto-update offered, so I ran Online > Check for updates - software is up to date.
Generated a fresh pdf file for NC only - - same result. Page-1 looks like this:

Page 1 is supposed to look like this (Fake Names, SSN's etc) .
Page 1 is supposed to be different from many other states forms....it has a computer scannable series of entries that NCDOR can run thru their scanner should a person have to mail in their NC tax forms. The same data points, with all the text, are on subsequent pages:
