I am a travel nurse with permanent residence in FL. I am going to take an assignment in DC for three months and my agency wants me to fill out a D-4 Form. Is that correct? or would I have to fill out a D-4A form since I’m not a DC resident? Thank you in advance!

Yes. If you are working in DC you will need to file and pay taxes on the income you earn from DC sources. This will be the same with any travel contracts you take in other states. You will need to pay non-resident taxes to the state for the money you earn in the state.
