The Boys and Girls club is a tax-exempt organization and can be used as childcare if you children are under the age of 13 and tihis was necessary for you (and your spouse if married) to work or look for work, and you (and your spouse if married) both have earned income.

What form would we need for the club to file?
The Boys & Girl’s Club of America is a tax-exempt organization. You do not need to file any form with your tax return, however you should get their EIN number.
There are many local divisions so get the right one. You should get a receipt or itemized bill showing how much you paid if you get audited. That should also have their number or they can give it to you when you visit.
