When you take a cash withdrawal from your 401(k) account, you'll receive a form 1099-R which you'll enter on your tax return.
You can enter your form 1099-R by following the instructions in this TurboTax Help article.
If you took money from a 401k you need to enter a 1099R. Get that from the retirement plan's website or import it from them if you can.
To enter your retirement income, Go to Federal> Wages and Income>Retirement Plans and Social Security>IRA 401 k) Pension Plan Withdrawals to enter your 1099R.
If your bank or financial institution is not on the drop down list, click the words “I’ll type it in myself” and key in your information by hand.
See if your financial institution is on the Partner List - If not, you will have to enter the information manually.

Yes, I haave a total of 4 1099-R called fidelity to clarify me.
1099-R code GB direct rollover ROTH
1099-R code G direct rolloever ROTH
1099-R code 1 early withdrawal (asumming 401K since i do not have nything else and they didnt say ROTH)
1099-R code 1B early withdrawal (asumming 401K since i do not have nything else and they didnt say ROTH)
I know I did the 1K hardship withrawal for the code 1 for 1192 (paying 192 in early tax) dont know if it makes a difference while filing but I just want to make sure
I actually called turbotax support and didnt get helped because the lady "hadn't seen the options before"
called fidelity they clarified the ROTH and codes but no tax advice

no, they are not correct. none have IRA box checked and none have FTCA filing box checked. some of the have total dist box checked. none have taxable amount not determined.
I was stuck at the “where is this distribution from?” And asked about gov company, 59 1/2, education and many other but I was able to skip without selecting. My question is, I skipped answering since” none applied “ but is that what I should do or one should be selected?
