You have to find out the date. Or if you bought it several times type Various. But you have to know if you held it Long Term or Short Term. If it is a combination of both you need to split the sale into 2 sales, LongTerm and Short Term.

And you need all the other info too. If nothing is on the form why are you entering a sale?
