Turbo Tax, for State Return, asks "Income of the Household member" to be entered. Is it the amount before Medicare is taken out or after?

It depends on which state tax return you are completing, and the reason for the question. Please provide that information, if you still need assistance.

For example, Minnesota has a special Property Tax Refund for which household income affects eligibility. See here for more information about how to calculate Minnesota household income for that purpose.
