My deceased husbands Trust received 1099-R with code 4 in box 7 - do I include the1099-R to my tax filing or his trust? I received the funds.

The 1099-R should be reported by the trust and the trust files and pays the tax (technically) before you receive the distribution. You are the beneficiary of the trust.
