Just leave all the other boxes blank or empty. Having the box checked will not cause any issues or conflicts with a lack of information in the other boxes. Its main purpose is to just open the screen to allow you to enter the additional information if necessary.
Yeah...sometimes, especially with imported files, the provider sometimes puts a bunch of unnecessary zeros in the extra boxes. That could be why it's always checked. We generally have folks just not enter anything in those extra boxes if the actual 1099-INT has those unnecessary zeros in them....but shouldn't cause issues unless a zero is in the state withholding box.
The only way to move beyond the error notification was to enter a “0” in this box, or it wouldn’t move forward with submitting.

Box 17 was highlighted in the error notification, so I could only get through this error notification and file my tax return was to enter “0” in this highlighted box. I could not leave it blank, even though it was blank on the form I uploaded. I did not check the box indicating other fields did have info and couldn’t remove this checkmark even by deleting the upload and uploading a fresh 1099-int form. Intuit has a software glitch that needs attention.
When I tested it...that didn't happen to me unless there was a state selected in box 15.
Then I got a warning about box 16....not quite the same as yours (I'm using Desktop) but it did go thru with box 17 set at zero.
But I didn't get a warning at all if boxes 15, 16, and 17 were all blank (Box 15 is blank if set on "-Select a State-")
All fields on all uploads (3) were blank. I did not check the box indicating info was present in those other blank fields. Intuit software put a check in that box and wouldn’t allow me to remove it. The error message that appears is highlighted box 17 and has curser inserted with keyboard appearing on my iPad. The only way to move past and resolve this error is to insert text/ number. I tried to insert a “-“ and couldn’t. I tried the same process on my Windows 11 laptop with the same failure. I was so frustrated I considered just downloading the IRS 1040SR form and filling it out from scratch, even though I paid Turbo Tax $79.95 for their bad advice to just proceed, leaving blank spaces and the process would move forward and file would submit despite this error/warning. It would not.
Next year I will I will save $79.95 and avoid using Turbotax!
