Insurance proceeds

I have Form OMB# 1545*0119 stating: gross distribution, taxable amount, tax withheld and a distribution code of 7D. Where do I enter this?

and when you enter the code, it is actually two separate codes

a 7

and a D

on two separate lines for box 7
