If my W-2 does not show my pre-tax traditional 401K contributions how and where do I enter my 401K pre-tax contributions?

If your W-2 is missing this information, you must obtain a corrected W-2 from your employer.  Elective deferrals to a traditional 401(k) are shown in box 12 with code D.
