If I worked a part time job for about 3 months and they didn’t take out state or federal taxes do I still include that as income?

Of course.  You must report all your income.  Even $1 of bank interest.  They don't take taxes out of interest and dividends and you have to report those.

Did they take out Social Security and Medicare?  Ask them if you are getting a W2 or a 1099Misc.  If you get a 1099Misc that is self employment income and you owe the Self Employment tax on it.   Wait for the form to come.
