You should get a 1099R from the 401K.  Did they take any withholding out of it?

There was no withholding from it

If it was a trustee-to-trustee direct rollover then the 1099-R should have a code "G".  No tax withholding is necessary for a direct rollover.  I assume the check was make out to your IRA and IRA custodian, not to you personally.

You still need to enter it on your 2017 tax return even though it will not be taxable.

Thanks! The check was not made out to me, it was made out to Wells Fargo where I opened the IRA account. I will make sure it has a "G" on it.
